Medical Financial Assistance

Our Medical Financial Assistance program provides temporary financial assistance to help qualified patients pay for health care based on their financial need. This includes emergency, urgent, or medically necessary care, as well as pharmacy services and products received from Kaiser Permanente providers or facilities.

Patients who qualify get some or all of their costs covered regardless of whether they have health care coverage, or are uninsured, or are underinsured.

Am I eligible for Medical Financial Assistance?

If your household income is at or below 300%, or in some regions up to 400%, of the  federal poverty guidelines , you may be eligible.
If you’re experiencing high medical expenses as compared to your income, regardless of household income, you may be eligible.

How can I get more program information?

What does the MFA program cover?

Emergency and medically necessary health care services
Pharmacy services and products
Medical supplies provided at Kaiser Permanente facilities (such as hospitals, medical centers, and medical offices), at Kaiser Permanente outpatient pharmacies, or by Kaiser Permanente providers

The MFA program does not cover:

Health care premiums
Health care services provided and billed outside of Kaiser Permanente
Ambulance or other medical transportation
Services that are not considered emergency or medically necessary as determined by a Kaiser Permanente provider, including cosmetic surgery and services, infertility treatments, retail medical supplies, and surrogacy services
Services related to third-party liability or workers’ compensation cases

How do I apply for Medical Financial Assistance?

The  most convenient option for most people is to apply online.

Alternatively, you can apply via fax, mail, or drop-off. 

For more details or to start the application online, see "Get Started" below and select the region where you received the services you need help paying for.

We will make every reasonable effort to process your application promptly. Once we receive the completed application, we’ll send you a letter confirming the outcome within 30 business days. If it is not approved, we will provide a reason for the denial and instructions on how to appeal the decision if you disagree.

Health care costs 
Proof of income

Income verification is part of determining eligibility for medical financial assistance. Including proof-of-income documentation with your application will help confirm your income accuracy during the review process. The table below lists the types of documents to submit.

Medical Financial Assistance award amounts are based on your household income as a percentage of the federal poverty guidelines :

Past MFA award recipients aren’t guaranteed future awards. Kaiser Permanente reserves the right to change the terms and conditions of the MFA program at any time. An MFA award is not a covered benefit and does not change the services covered or cost-sharing owed under your plan.

How do I file an appeal?

If your application was recently denied or you believe you qualify for a larger award, you can appeal the decision.

Start an appeal application by providing your full name and email address in the form . Once submitted, you will receive instructions via email on how to file an appeal.

You will need to provide:

The reason why you’re appealing (job loss, for example)
Current financial documentation of your total gross household income to support your appeal (refer to the table shown in the “ What information will I need to apply ” section above for the required documentation of your income)

You may also file an appeal if your medical expenses exceed 10% of your annual gross household income over the previous 12 months.

To do this, you should provide the following:

Proof of income for your annual gross household income
Proof of non-Kaiser Permanente medical and/or dental expenses
Provider’s name and address
Patient’s name
Description of services or supplies provided

You will not be refunded for any payments you make before an appeal is approved.

You have 30 days to file an appeal. After 30 days, you will be asked to submit a new application.

To appeal for a larger award, you may appeal at any time before the award end date.

If you didn’t provide the requested financial documentation or if your income changed, you can submit additional documentation. See the “ What information will I need to apply ” section above for the required income documentation.

Once we receive the requested documentation, we’ll send you a letter regarding the decision on your appeal within 30 days. If you file an appeal without including any additional information with your request, we’ll automatically deny your appeal. All appeal decisions are final.

How much will patients who have no health insurance be charged for services? (Amounts generally billed)

" Amounts generally billed " or AGB, is based on the lowest average of the amounts that were paid to a Kaiser Permanente facility by private health insurers, Medicare fee-for-service and Medicaid fee-for-service (and co-pays and deductibles) for emergency or medically necessary services, based on actual claim data from October 1, 2022, to September 30, 2023, which is consistent with the look-back method. Total expected payment from allowed claims was divided by total billed charges for such claims, and that number was subtracted from 1 to calculate the AGB percentage. The 2024 AGB reduction to gross charges is 67% for hospital charges.

The Hospital Bill Complaint Program is a state program, which reviews hospital decisions about whether you qualify for help paying your hospital bill. If you believe you were wrongly denied financial assistance, you may file a complaint with the Hospital Bill Complaint Program. Here’s an overview of getting care with Choice Products. In the Added Choice 2 Tier Point-of-Service (POS) plan, there are two Tiers to choose from: In-Network Provider Tier and Out-of-Network Provider Tier.

In-Network Provider Tier

Out-of-network provider tier, log on to kp.org, kaiser permanente providers, features and benefits of our plan..

Lowest out-of-pocket costs. With this Tier, you’ll usually pay the least out of pocket, with no or a lower deductible.
Coordinated care. Your primary care physician works closely with specialists, pharmacists, lab technicians, therapists, and many other professionals. And because they are all connected through your electronic medical record, everyone in the network gets up-to-the-minute information on your health for a better care experience.
Referrals from your doctor. Your primary care physician can coordinate your referral to a specialist, and can even schedule your appointment.
State-of-the-art medical offices. Kaiser Permanente’s state-of-the-art medical offices are located throughout the region, with doctors, specialists, imaging, and in most locations, pharmacy and lab, all in one place.
Emergency care. You are covered for emergency care worldwide. The emergency care copay will be waived if you are directly admitted to a hospital as a result of an emergency.
Urgent care. Urgent care is available at Kaiser Permanente medical offices. Call the number on the back of your ID card to confirm locations and hours of operations.
Away from home. Visit kp.org/travel or call the away from home travel line at 1-951-268- 3900 for all of your options.
Hospital care. Kaiser Permanente carefully selects hospitals to partner with us in taking great care of you.*
Medical advice. When you call for medical advice you will be connected with a professional that can help advise and make appointments with one of our providers. You can get advice by calling the number on the back of your ID Card. 24/7.
Lab services. Your results from tests done in Kaiser Permanente medical offices can be read (for most results) online soon after the lab completes your tests, sometimes the same day.
No-cost preventive care. With your plan, you pay $0 for preventive care. That includes routine physicals, well-child visits, and certain screenings and tests (such as mammograms).
Choose from telehealth options that fit into your schedule. Chat with a Kaiser Permanente doctor, email, or schedule a phone or video visit. Be aware that costs may apply for some visits.**

Features and benefits of receiving covered care from any licensed provider..

Provider choice, care anywhere. Your Out-of-Network Provider Tier benefits cover care you receive from any licensed provider (who’s not in the In-Network Provider Tier).
Using Out-of-Network Providers generally means higher out-of-pocket costs. Your out-of-pocket costs will generally be the highest when you choose to receive care from an Out-of-Network Provider or facility.
Behavioral health. You can receive care from any licensed behavioral health or chemical dependency professional for mental illness, emotional disorders, and drug or alcohol abuse.*
Hospital care. You can receive inpatient hospitalization services from licensed or accredited hospitals and facilities not in the In-Network Provider Tier.*
Emergency care. You can visit any licensed Out-of-Network emergency care facility. Emergency care is covered at the In-Network Provider Tier benefit level, and you will be responsible only for the In-Network cost share, regardless of where you seek care.
Urgent care. You can visit any licensed Out-of-Network urgent care facility.
Maternity care. You can choose any licensed provider for obstetric care.*
Lab tests, X-rays. You can contact facilities directly. You can request your Out-of-Network Provider send labs, X-rays, and special procedures (MRIs, CT scans, PET scans, nuclear medicine), to a Kaiser Permanente facility, In-Network Provider, any time you choose, and you will likely pay less.

Depression, anxiety improve in patients enrolled in care personalized to their symptoms

Kaiser Permanente study finds symptoms improved in both a “collaborative care” model and typical psychiatric care

A study comparing traditional psychiatric treatment of depression and anxiety with a team-based collaborative care approach found that patients in both models saw significant improvement of their symptoms, but the improvement was even stronger in the collaborative care approach. The study was published in the Journal of Clinical Psychiatry.

Team-based collaborative care has been offered to Kaiser Permanente Northern California (KPNC) patients since 2020. Care is focused on frequent symptom monitoring and treatment personalized to patient symptoms, with care provided by therapists with support from pharmacists. The study covered a period early in the COVID pandemic when most care was being delivered virtually through telehealth.

Patients in the collaborative care group with depression saw an average 9-point improvement as measured by a 27-point patient health questionnaire, a validated tool that can be used to measure patient depressive symptoms at each visit. There was a 5-point improvement in the traditional care group.

The improvement in both groups was notable, particularly because care took place largely through phone and video visits, said lead author Kathryn Erickson-Ridout , MD, PhD, a psychiatrist with The Permanente Medical Group and adjunct investigator with the Kaiser Permanente Division of Research . “The data shows that in both programs, patients with mild to moderate symptoms improve,” she said. “In fact, a high proportion of depression patients reached full remission of their symptoms in both programs — 27% in specialty care and 43% in collaborative care, when the national average is around 10%.

“The average depression patient in this study moved from moderate symptoms to not meeting criteria for depressive disorder,” Erickson-Ridout added. “That kind of improvement is great to see because depression remission can offer improved functioning and less risk of relapse.”

Erickson-Ridout was also encouraged by what the results suggest about the collaborative care being used by KPNC. Collaborative care is used nationally and features patient-centered care from a team of providers, regular evaluation of patient symptoms and status, and treatment to a symptom target using evidence-based practices. The KPNC version adds monitoring the patient’s progress and recommending any additional interventions into every visit, medication management by program pharmacists, and integration of symptom tracking into existing clinical tools.

Another benefit of collaborative care, she said, is that therapists and pharmacists assist the psychiatrist in a team approach that can address the national shortage in mental health clinicians.

The study compared 1,607 mild-to-moderate depression patients in collaborative care with 8,773 patients in specialty psychiatry, and 570 mild-to-moderate anxiety patients in collaborative care with 2,365 in specialty psychiatry. Their symptoms were tracked for 6 months between 2020 and 2021.

The collaborative care depression patients saw a 9-point decrease in symptoms as measured on the 27- point Patient Health Questionnaire-9, and collaborative care anxiety patients saw a 5.4-point reduction in symptoms as measured on the 27-point Generalized Anxiety Disorder-7 Scale. For those in specialty psychiatry care, depression symptoms went down 5 points and anxiety symptoms down 2.8 points.

“This initiative exemplifies KPNC’s Behavioral Health service line’s continuous dedication to implementing evidence-based interventions like collaborative care on a larger scale,” said co-author Maria T. Koshy , MD, a psychiatrist with The Permanente Medical Group and director of regional mental health administration. “It also contributes to the expanding body of research supporting multi-disciplinary, team-based psychotherapy and medication management programs, aimed at providing patients with depression swift and significant symptom relief.”

Collaborative care option growing

KPNC patients who seek treatment for mild to moderate depression or anxiety symptoms through their primary care doctor or directly with psychiatry choose either collaborative care or specialty psychiatric care with assistance from their doctor and mental health clinicians.

Specialty psychiatric care involves meeting with a psychiatrist, who develops a treatment plan in partnership with the patient based on their clinical judgment and may or may not include measurement-based care.

Some KPNC patients may also have their mild-to-moderate depression or anxiety treated by their primary care physician if they prefer, and some KPNC medical centers have primary care-based collaborative psychiatric care programs.

Previous research has compared collaborative psychiatric care models to mental health managed within primary care. This is the first study to compare a collaborative model with specialty psychiatric care, Erickson-Ridout said.

The research is also unique in providing evidence about how these programs compare when care is provided virtually. While a lower proportion of KPNC patients are getting phone or video psychiatric care in the years since pandemic restrictions ended, a significant percentage still use virtual care.

“Since this virtual collaborative care program’s inception in April 2020, we have treated more than 37,000 patients with depressive and anxiety disorders,” said Lisa Fazzolari , DO, a TPMG psychiatrist and associate chair of psychiatry. “The telehealth platform allows for treatment to be easily accessible and convenient for large populations of patients. At KPNC, we have successfully implemented population management programs to treat patients with other chronic conditions such as diabetes, hypertension, and heart disease and are now applying this care approach to mental health.” Fazzolari noted that a new pilot program is testing collaborative care for depression and anxiety around the time of pregnancy.

Novokuznetsk is the largest city in the Kemerovo region of Russia. It is located in the south of Western Siberia, on both banks of the Tom River, in the southern part of the region. It is an important economic, transport and cultural center of Siberia.

History of novokuznetsk.

Novokuznetsk is one of the oldest cities in Siberia. It was founded approximately in 1617-1618. From 1620 till the end of the 18th century, it was a stockaded town located on the right bank of the Tom River.

Present Novokuznetsk was formed on July, 3, 1931, on the basis of the settlement of Sad-gorod (from 1929), near a metallurgical plant. In March 1932, it included the town of Kuznetsk and was renamed into Stalinsk in May 1932. In the 1930s, industrialization turned the town into an important coal-mining and industrial center.

Kuznetsky metallurgical plant began operating in 1931. Later, in 1941, two evacuated plants were also put into operation in the city: Novokuznetsky aluminum plant and Kuznetsky iron-alloys plant. Kuznetsky metallurgical plant produced military products, several tank armies were produced of steel produced in Novokuznetsk.

Later, in 1961, the West-Siberian metallurgical plant was constructed. November 5, 1961, Stalinsk was renamed in Novokuznetsk. According to the census conducted in 1989, the population of the city was 599,900.

By 1998, the city was in decline. The crisis of 1998 led to the devaluation of the ruble and coal mining and ferrous metallurgy became profitable again. The economic growth of the 2000s caused rapid construction in the city.

Novokuznetsk is one of the largest metallurgical and coal production centers in Russia, although most of the mines on the territory of the city are closed.

The distance from Moscow by roads is 3,751 km. Novokuznetsk City Day is celebrated on the first Sunday of July. It is the largest city after Tolyatti that doesn’t have the status of an administrative center of the region.

Novokuznetsk is characterized by sharply continental climate. The average temperature in January is minus 15.2 degrees Celsius, in July - plus 18.8 degrees Celsius.

From Novokuznetsk railway station you can go by train to Moscow, St. Petersburg, Novosibirsk, Vladivostok, Barnaul, Tomsk, Abakan and other cities, as well as to Kazakhstan and Kyrgyzstan. International Airport Novokuznetsk (Spichenkovo) offers flights to Moscow, Krasnoyarsk, Bangkok (Thailand), Kamran (Vietnam).

The main industries of Novokuznetsk: ferrous metallurgy, coal mining, non-ferrous metallurgy, power engineering, machine-building, and metal-working. The environmental situation is unfavorable in the city. Air pollution is mainly caused by the metallurgical plants.

Novokuznetsk

It was previously known as Kuznetsk until 1931, and as Stalinsk until 1961.

Founded in 1618 by men from Tomsk as a Cossack ostrog (fort) on the Tom River , it was initially called Kuznetsky ostrog ( Кузне́цкий острог ). [2] It became the seat of Kuznetsky Uyezd in 1622. [3] Kuznetsk ( Кузне́цк ) was granted town status in 1689. [3] It was here that Fyodor Dostoevsky married his first wife, Maria Isayeva in 1857. [13] Joseph Stalin 's rapid industrialization of the Soviet Union transformed the sleepy town into a major coal mining and industrial center in the 1930s. It merged with Sad Gorod in 1931. From 1931 to 1932, the city was known as Novokuznetsk and between 1932 and 1961 as Stalinsk ( Ста́линск ), after Stalin. As a result of de-Stalinization , it was renamed back to Novokuznetsk .

Within the framework of administrative divisions , Novokuznetsk serves as the administrative center of Novokuznetsky District , even though it is not a part of it. [1] As an administrative division, it is incorporated separately as the Novokuznetsk City Under Oblast Jurisdiction —an administrative unit with the status equal to that of the districts . [15] As a municipal division , Novokuznetsk City Under Oblast Jurisdiction is incorporated as the Novokuznetsky Urban Okrug . [15] [16]

On 7 December 2009, by a resolution of the Novokuznetsk City Council of People's Deputies, a new version of the City Charter was approved, [16] according to which the authorities consist of:

The Novokuznetsk City Council of People's Deputies is a representative body of power and consists of 18 deputies elected in 18 single-mandate constituencies and 18 deputies elected on party lists. The term of office of deputies is five years.

In September 2021, elections were held for the Council of People's Deputies, following which the seats in the council were distributed as follows: 27 - United Russia , 2 - Liberal Democratic Party , 2 - A Just Russia , 2 - Communist Party . The representative of United Russia, Alexandra Shelkovnikova, was elected chairman.

The Youth Parliament of the city operates under the City Council of People's Deputies. [19]

A whole network of bodies of territorial public self-government has been created in Novokuznetsk; [20] in total, 60 of them have been created in the city: in the Zavodskoy District - 7, Kuznetsky - 6, Kuibyshevsky - 13, [21] [22] [23] Novoilyinsky - 6, Ordzhonikidzevsky - 9, Central - 16.

Promyshlennovsky District is an administrative district (raion), one of the nineteen in Kemerovo Oblast, Russia. As a municipal division, it is incorporated as Promyshlennovsky Municipal District . It is located in the west of the oblast. The area of the district is 3,083 square kilometers (1,190 sq mi). Its administrative center is the urban locality of Promyshlennaya. Population: 50,106 (2010 Russian census) ; 50,125 ; 47,150 (1989 Soviet census) . The population of Promyshlennaya accounts for 36.0% of the district's total population.

Topkinsky District is an administrative district (raion), one of the nineteen in Kemerovo Oblast, Russia. As a municipal division, it is incorporated as Topkinsky Municipal District . It is located in the northwest of the oblast. The area of the district is 2,774 square kilometers (1,071 sq mi). Its administrative center is the town of Topki. Population: 16,246 (2010 Russian census) ; 18,077 ; 19,018 (1989 Soviet census) .

It is located in the northeast of the oblast. The area of the district is 3,531 square kilometers (1,363 sq mi). Its administrative center is the urban locality of Tyazhinsky. Population: 25,597 (2010 Russian census) ; 32,782 ; 32,574 (1989 Soviet census) . The population of the administrative center accounts for 43.4% of the district's total population.
